Insert add hours dialog into ticket template rather than having a sidebar

Rather than having the add hours sidebar on the ticket page, we'll inject the add hours into the ticket template, like the DuplicateTicketSearchPlugin and TracTicketChangelogPlugin do.
The TicketSidebarProviderPlugin dependency will be removed.

Which Trac version are you running? If I implement the change, I will probably only make it work with Trac 1.0 and later.

We are still on 0.12.3. But hope the provider will upgrade.

Want to let anyone concerned know that this plugin looks like it is more useful than the "Timing and Estimation" plugin for us. However, this issue of this ticket makes it a show stopper for us. We spend a lot of time editing tickets, and editing things off to one side of the screen is a pain-in-the-neck (literally).
