Additional template admin_milestones.html as default

By default admin_milestones.html is not included in the 'insert_into' (ie no calendars if you go to admin - milestones instead of via roadmap).

I think it should either be included as default, or documented explicitly.

Thank you for this hint. I just inserted the fields where I needed them the most and left it to everybody to add fields in their config.
